Reason 1: You rely too much on word of mouth (and not enough on local SEO)
It’s always such a great feeling when a new client comes to you and says, “My friend said I had to see you!” or “My Dad said you really helped him with his back.” Word of mouth is brilliant, and it’s great when your diary is full, but it’s not a strategy you can control or scale. When a gap appears in the diary, there’s no digital safety net.
The most common pattern I see is a clinic or wellbeing business that’s been busy for years, assumed digital marketing was something they’d “get to later” … and then later arrived faster than expected! Maybe a new practice opened nearby, or a key GP referral dried up, and suddenly the phone wasn’t ringing like it used to, and you realise you couldn’t be seen on Google very well.

The fix: Claim it, complete it, and keep it active. Include your services, photos, opening hours, FAQs, and – critically – respond promptly to reviews. Google rewards active profiles with better local visibility and a steady trickle of recent reviews can get you appearing in local map results without any paid advertising. A simple follow-up message asking for a review after a course of treatment can make a big difference to rankings… and a new patient’s decision to book. Reason 3: Your clinic website isn’t built for mobile
Most health-related searches happen on a mobile phone – often in the evening, sometimes by someone in discomfort who wants to book quickly. If your website is slow to load, hard to navigate on a small screen, or doesn’t have a clear online booking option visible within seconds, that person is gone.
The fix: A mobile-first website that loads fast, looks clean, and makes booking easy. This doesn’t necessarily mean a full rebuild; sometimes it’s targeted fixes. The test I always suggest that you can do right now is very simple. Pull out your phone, open your browser, and try to book an appointment on your own website. If you find it frustrating, your patients definitely do. And unlike you, they won’t persist!

The fix: You don’t need to become a data analyst. But you do need a monthly habit of checking a handful of key numbers: sessions, traffic source, most-visited pages, and conversions. Knowing what’s working means you can do more of it. Knowing what’s not means you can fix it.
I always look at traffic volume, traffic source and then the conversion. Conversion could be a form submission, a phone number click, or a booking. If traffic is healthy but conversions are low, the problem is the website. If traffic is low, the problem is visibility.
